In October 2025, AIKEY deployed a pilot GuardView physical AI safety system at a Youngshine Supo heavy construction site located in Seoul, South Korea. The core objective of the pilot system was to transform raw site footage into structured and actionable “Safety Intelligence”, validating the AIKEY value statement and expected benefits with objective field evidence.
2. Analysis of The Actual Work Site
During the five-day window, 9 events were automatically captured and uploaded to SkyView. No manual logging was required. Events were sorted by priority (critical, moderate, and low). Each event is accompanied by the actual SkyView CCTV image capture, a structured problem and corrective action summary, the applicable Korean safety regulations, and, when applicable, a related real-world accident case that illustrates the risk consequence of the same type of failure.

EVENT 1 – 03/17/26 09:30
WORKER UNDER SUSPENDED MATERIALS. FALLING OBJECT CRUSH RISK
Problem Observed
A worker was observed moving into the crane hoisting radius while a load was being lifted. If the load shifted or rigging failed, the worker would have been struck with fatal force. No dedicated signaler was present to control zone access.
Corrective Actions
Dedicated signaler assigned to all hoisting operations. Physical safety fence and access prohibition signs installed near hoisting radius. Worker entry ban registered in AIKEY Rule KB and communicated to all site workers.

EVENT 2 – 03/20/26 14:25
SAFETY NET DETACHED AND UNSECURED. ACTIVE FALL PROTECTION FAILURE
Problem Observed
SkyView captured a dangerous and preventable five-step sequence over 90 seconds. A connector detached during a two-person shoveling task, a net was left open after one worker exited, the collapsed net was used while work was still underway, and the net was dragged without repair. An open void with no fall protection existed in work zone.
Corrective Actions
All work is stopped on any tasks that affect facility safety until the proper steps are taken. The safety net was immediately reinstalled to specification under the site manager’s approval. Moving forward, temporary removal of the net now requires prior written approval and a designated monitor.

EQUIPMENT MAINTENANCE WITH NO SIGNALER. WORKER ENTRAPMENT RISK
Problem Observed
Maintenance was performed on an excavator while the operator remained in the cab with the engine potentially runnable. No traffic controller was present. Workers were inside the machine's rotation zone with no alert system allowing a crush or entrapment incident to occur without warning if the machine moved.
Corrective Actions
The Operator must exit the cab and remove the ignition key during all maintenance. If the engine must run, all control levers must be moved to the locked position. Traffic controller must be assigned, and the zone must be demarcated with cones.

EQUIPMENT MAINTENANCE WITHOUT A HARD HAT.
FALLING OBJECT AND COLLISION RISK
Problem Observed
A worker was observed performing equipment maintenance without wearing a hard hat in an active construction zone. This creates risk of injury from falling objects, collision with equipment, and trip falls. Korean OSH law requires PPE provision and enforcement by the employer.
Corrective Actions
Hard hat mandatory for all workers in any equipment proximity zone without exception. Supervisor spot-checks introduced at shift start and during operations. Non-compliance tracked in AIKEY Safety Event DB.

EVENT 2 – 03/16/26 12:35
FIRE, EXPLOSION, AND OCCUPATIONAL HAZARDS IN WELDING OPERATION
Problem Observed
Welding was conducted with no spatter protection screen. Additionally, the gas cylinder was not properly secured against toppling; multiple workers were within the arc flash and fume exposure radius; and the welder's own body protection was insufficient. Combined fire, explosion, and occupational health risk were present simultaneously.
Corrective Actions
Spatter protection screen must be installed before all hot work. Gas cylinders must be properly secured with chains. Non-essential workers are to be excluded from the exposure zone. A qualified fire watcher must be assigned. Full protective equipment is mandatory for all welders, and the designated hot work permit system must be enforced.

EVENT 3 – 03/16/26 11:27
COMBUSTIBLE CONTAINER ADJACENT TO SMOKING ZONE
: FIRE AND HEALTH RISK
Problem Observed
The smoking zone was incorrectly designated directly beside a combustible material container used as a rest area. Waste and scrap materials were piled around the container and blocking fire extinguisher access. Workers inside were exposed to passive smoke and the surrounding clutter created multiple trip and fall hazards.
Corrective Actions
Hard hat mandatory for all workers in any equipment proximity zone without exception. Supervisor spot-checks introduced at shift start and during operations. Non-compliance tracked in AIKEY Safety Event DB.

EVENT 4 – 03/20/26 16:11
WORKER IN EXCAVATOR BLIND SPOT WITH HOSE TRIP HAZARD ON WALKWAY
Problem Observed
A worker was observed moving through the blind spot of a running excavator (tracks visible and moving). A hose was also coiled across the walkway in the same area. Combined risk: Equipment entrapment and trip and fall injury in the same zone simultaneously.
Corrective Actions
A blind spot exclusion zone was physically demarcated around all operating excavators. Hose management protocols were added to the daily housekeeping requirements. Walkway clearance must be verified at the start of all shifts. Blind spot violations to be tracked in AIKEY Safety Event DB.

EVENT 5 – 03/21/26 07:32
PILE DRIVING SILICA DUST: WORKERS UNPROTECTED, NO WATER SUPPRESSION
Problem Observed
Pile driving activities generated heavy sand and silica dust. Adjacent workers were clearly visible inside the dust cloud without appropriate respiratory PPE. Additionally, no water suppression was applied during the operation. Prolonged silica exposure causes irreversible respiratory disease; even short unprotected exposure during heavy dust generation is considered a significant health hazard.
Corrective Actions
Water suppression must be implemented during all pile driving activities, with work to be paused if a functioning system is unavailable. Respiratory PPE is mandatory for all workers within 15 meters of the dust source. Dust exposure zone was registered in AIKEY Safety Event DB for ongoing, continuous health surveillance purposes.

CELL PHONE USE WHILE WALKING THROUGH ACTIVE WORK ZONE
Problem Observed
Workers were observed using cell phones while in transit through an active construction zone. Phone use while walking diverts attention from ground hazards, moving equipment, and other workers thereby creating trip, collision, and inattention risk.
Corrective Actions
Cell phone use is prohibited while walking on the worksite. Designated stationary phone-use areas have been defined. Policy was communicated and posted at all site entrances. Repeat violations are being tracked in AIKEY Safety Event DB.
